The Account Holder agrees to the
following terms and conditions:
Cash withdrawals are prohibited from the Declining Balance Account.
The Declining Balance Account can be used at the Bookstore, Tiger Inn, and
Coffee Shop, or to purchase tickets to CAC (College Activity Committee)
Parents and/or students are responsible for determining, and enforcing any
purchasing restrictions on the account.
Student must present a valid student identification card to purchase
against this account--no exceptions.
A lost, stolen, or damaged identification card must be immediately
reported to the Hampden-Sydney Bookstore.
Parents may add funds to the account by check (made payable to the Hampden-Sydney Bookstore), OR online with this form and credit card (American Express, Visa, MasterCard, or Discover) and the student's ID number.
Merchandise returned for refund will be credited back to the account. Cash
back transactions against the account are not permitted.
Any balance remaining at the end of a semester will automatically be
rolled over to the next semester.
Money deposited to the account will be refunded only upon graduation,
separation, or withdrawal from the college.
Please provide the following information: incomplete applications
cannot be processed. * Required
